Cornerstone Down Syndrome Care provides support
for children from the prenatal stage through age 18.
Initial Assessment
1 hour with caregiver, via telehealth or in-person
Parental/Caregiver interview to assess concerns
Provider or medical team concerns
Collaborative goal setting
Secondary Assessment
30 min to 1 hour with child based on parental preference
Child evaluation to include general assessment
Assessment regarding specific concerns
Assessment and Recommendations
1 hour with caregiver
Compassionate expert knowledge sharing in regards to the care of their child
Discussion of preventative health guidelines
Discussion of any caregiver medical and non-medical concerns
Sustained Care and Support
Ongoing health management
Continued developmental surveillance and guidance
Discussion of preventative health at every age
Discussion of any caregiver medical and non-medical concerns at every stage
